WELCOME TO HEALTH SERVICES
Hi, my name is Sheryl Kunz and I am the School Nurse at Mascoutah Elementary. I am a Registered Nurse with 16 years of nursing experience. I am here to provide your children with their health, first aide, and medication needs. I am available Monday through Friday, 8 a.m. to 4 p.m.
Beginning with the 2016-17 school year:
The school board has adopted Policy 7:100 that now requires all students entering Pre-K, Kindergarten, 6th and 9th grades to provide new and updated physical and immunization records by the first day of school. Students in grades 6 and 12 must provide proof of immunization against meningococcal disease. If the required documentation is not provided, your child will be excluded from school attendance the first day of school.
Asthma Action Protocol
The State of Illinois Public Act 099-0843 requires schools to request parents of students with asthma to provide an Asthma Action Plan from the student's healthcare provider. This action plan will be kept on file with the school nurse. This Action Plan needs to be submitted at the start of each school year.
