Beginning with the 2016-17 school year:
The school board has adopted Policy 7:100 that now requires all students entering Pre-K, Kindergarten, 6th and 9th grades to provide new and updated physical and immunization records by the first day of school. Students in grades 6 and 12 must provide proof of immunization against meningococcal disease. If the required documentation is not provided, your child will be excluded from school attendance the first day of school.
Illinois Health Physical Form-All Pre-K and Kindergarten students must have a recent Physical. Students new to Illinois must have a physical completed on a State of Illinois Physical Form. Students should be up-to-date on immunizations (bring shot record)

Dental Exam-Forms must be completed for students in grades Kindergarten and 2nd grade.
A dental examination form completed by the dentist, or waiver, if the student qualifies for one of four criteria.

Vision Exam-Vision exams are required for all students entering Kindergarten or an Illinois school for the first time. A physician licensed to practice medicine in all of its branches or a licensed optometrist must perform the required eye examination.
